1. All food vendors must complete and mail the Department of Health Temporary Food Service Application to Molokai Health Office, P.O. Box 2007, Kaunakakai, HI 96748.
2. Complete the Pineapple Festival booth application. Completed Application Packets (all vendors) must be turned in by Tuesday, June 14, 2011 with your booth payment. No exceptions will be made.
3. Please state what you will be selling, displaying or demonstrating. If for sale, please state price. It is important to have this part completed. Signs will be provided by LCA with sale items and price.
4. Booth will be assigned as payment comes in.

One (1) 10’ x 10’ booth space will be provided per booth. Food vendors will need to provide their own tent and is allowed to install one (1) additional 10x10 tent at the back of the booth or coverings may be added to the booth. Due to the limited number of booth space available, each vendor will supply their own tables and chairs. Electricity will be provided. No open cooking within 20’ of tent.ALL FOOD VENDORS MUST HAVE A CERTIFIED FIRE EXTINGUISHER AT THEIR BOOTH. One (1) table and two (2) chairs are available to rent for $15. Food Vendors, in addition to your booth fee of $150.00 there will be a $25.00 fee for your Temporary Food Permit. Applicants are responsible to request for your food permit. Once you receive your food permit from the State of Hawaii, please submit your Booth Application with a copy of your approved permit. A deposit of $100 will be collected from the food vendors to ensure that no oil, food particles or trash is thrown on the ground and in the sinks. If food vendors do not comply with the rules, and oil, food particles or trash is found on ground or in sink, the deposit will NOT be returned.
All food must be prepared in a State Certified Kitchen, which will be provided at the E Malama I Na Keiki O Lanai Preschool. Please contact the school at 565-6266 to reserve a time to use the kitchen, the cost for the use of the kitchen is $25.00, all applicants must comply with the certified kitchen policy or you will not be allowed to sell at the event.
Vendors participate at their own risk and must read and sign the indemnification form. Please sign below to verify that you understand the above requirements for booth space.

ACKNOWLEDGEMENT AND INDEMNIFICATION
Vendor/Operator shall defend, indemnify and hold Lanai Community Association, it’s officers and members from and against all loss, liability, claims and demands for injury, damages, including but not limited to claims for property damage, and shall reimburse Lanai Community Association for its costs and expenses, including reasonable attorney fees, in connection with and defense of any such claim.
Vendor/Operator further agrees that in case Lanai Community Association shall, without any fault on its part, be made a party to any litigation commenced by or against the undersigned, then it will defend Lanai Community Association in any such litigation and will pay all costs and expenses be incurred by or imposed on Lanai Community Association by or in connection with such litigation.
