1.      All food vendors must complete and mail the Department of Health Temporary Food Service Application to Molokai Health Office, P.O. Box 2007, Kaunakakai, HI  96748.

2.      Complete the Pineapple Festival booth application. Completed Application Packets (all vendors) must be turned in by Tuesday, June 15, 2010 with your booth payment.  No exceptions will be made.

3.       Please state what you will be selling, displaying or demonstrating.  If for sale, please state price.  It is important to have this part completed.  Signs will be provided by LCA with sale items and price.

One (1) 10’ x 10’ booth space will be provided per booth.  Food vendors are allowed to install one (1) additional 10x10 tent at the back of the booth or coverings may be added to the booth.  Due to the limited number of booth space available, each vendor will supply their own tables & chairs.  Electricity will be provided.  No open cooking within 20’ of tent.ALL FOOD VENDORS MUST HAVE A CERTIFIED FIRE EXTINGUISHER AT THEIR BOOTH.
